Parental Permission
“Written consent is necessary if an activity involves travel outside the local area (as determined by local leaders) or staying overnight. Leaders may also request written consent for other activities when they feel it is appropriate.”
“Parents and guardians give this consent by signing the Parental or Guardian Permission and Medical Release form.”

There are two options for filling out this form using the link from this site.
- The print version is printed and then filled out and signed.
- The interactive version allows the form to be filled out on the computer, however it will still need to be printed in order to be signed.
A third option is to pick up an already printed form from your YM or YW leader.
Either way, the form must be physically signed.
Turn in the signed form to your ward youth leader: Young men to your Ward Young Men President, Young Women to your Ward Young Women President.
Once the ward YM/YW president has collected all the signed forms from their participating youth, these forms need to be turned in to a member of the Stake Young Women or Young Men Presidency by the registration deadline.
A separate, signed Permission and Medical Release Form is required for a youth to participate in Stake Youth Conference or Stake Young Women Camp. The same form may NOT be used for both activities.
